Reimbursement for Internet Access
ISP Reimbursement - To be reimbursed for your Internet access expenses (up to the cost of the lowest level of broadband service available in your area) please download and print the ISP Reimbursement Form. The current billing period is from January through May 2008. Then submit by mail, fax or bring in the completed form, along with your receipts for Internet service from January through May 2008 to:

Please note:  You must send in your original billing statements. Please keep a copy of the original statement for your records. No reimbursements can be issued without submiting your billing statements. No exceptions will be made to this policy.

All reimbursement forms must be turned in by June 15th, 2008.
